Can you send us your Skyline document?
In Skyline you can use the menu item:
File > Share
to create a .zip file containing your Skyline document and supporting files including extracted chromatograms.
If that .zip file is less than 50MB you can attach it to this support request.

In order for the light:heavy ratio to work, there needs to be a light and heavy precursor under the same molecule.
In your document, you have two separate molecules where one molecule has a light precursor and the other molecule has a heavy precursor.
I've attached a document where there is one molecule with a light and heavy precursor.
In your transition list, you should make sure that the heavy and light precursors have the same molecule name.
It looks like in your transition list the light precursor was named "Cortisol." and the heavy precursor was named "Cortisol-d4.".
If those lines had had the same molecule name instead then I think things would have worked for you.
